Medical Social Worker (Palliative Care)

Roles & Responsibilities:

• Deliver emotional, social, and practical support to patients and their caregivers, including holistic needs assessment, case management, counselling support, and outreach/home engagement.
• Coordinate care plans with multidisciplinary teams to ensure integrated support for patients’ well-being.
• Establish and maintain effective collaboration with external social service partners, healthcare institutions, and relevant agencies.
• Conduct psychosocial evaluations and provide appropriate intervention strategies based on individual and family needs.

Requirements:

• Degree in Social Work or equivalent qualification
• Full registration with the Singapore Association of Social Workers
• At least 3 years of experience within a hospital, clinical, or healthcare-related environment.
• Strong commitment to patient-centric care and community support.
• Adaptable, resilient, and comfortable working across diverse and dynamic settings.

Salary: $4,000 to $6,800
